Help with shoulder pain when reaching, lifting or sleeping

The shoulder is the most mobile joint in the body, which makes it prone to wear and inflammation. If reaching up, dressing or lying on your side is painful, a targeted injection alongside physiotherapy may help.

Shoulder pain can make everyday tasks such as dressing, reaching up or sleeping on your side difficult. It usually comes from the joint itself, the tendons and bursa around it, or the small joint at the top of the shoulder.

Dr Salama uses ultrasound to guide injections to the structure thought to be causing the pain. For pain that has not settled with physiotherapy, or where surgery is not suitable or surgical criteria are not met (for example, a small rotator cuff tear), nerve treatments such as suprascapular nerve block and pulsed radiofrequency may give longer-lasting relief.

Getting help

When to see a specialist

Consider a specialist opinion if you have:

  • Shoulder pain lasting more than 6–12 weeks despite physiotherapy
  • Pain that disturbs your sleep
  • Stiffness that is limiting daily activities
  • You have been told surgery is not suitable, or want to avoid it
Treatment

Treatment options

Shoulder injections are performed under ultrasound guidance, which helps place the needle correctly without X-ray radiation. Steroid injections usually give short-term relief and work best when combined with a physiotherapy programme.

Shoulder joint (glenohumeral) injection

Subacromial bursa injection

Acromioclavicular (AC) joint injection

Biceps tendon sheath injection

Suprascapular nerve block, sometimes combined with a circumflex (axillary) nerve block in selected cases, and pulsed radiofrequency

Common questions

Frequently asked questions

How many steroid injections can I have in my shoulder?

Injections are generally spaced at least three months apart, and the number is limited to protect the tendons and joint. Dr Salama will discuss what is appropriate for you.

Can injections help frozen shoulder?

A joint injection in the painful early phase may reduce pain and make physiotherapy easier. Some people also benefit from a suprascapular nerve block.

How soon will a shoulder injection work?

The local anaesthetic may ease pain within minutes. The steroid usually takes a few days to start working, and the effect varies from person to person.

Do I need a GP referral?

A GP referral is welcomed but not always required. Some insurers need one, so check with your insurer if you are claiming.

Can I use my private health insurance?

Yes. Dr Salama is recognised by most major private medical insurers and is Bupa Fee Assured. Check with your insurer whether you need pre-authorisation. Self-funding patients are also welcome, and consultation fees are published on this site.

How quickly can I be seen?

Appointments are usually available within 2–5 days of getting in touch.
