Could your back or neck pain be coming from the facet joints?

The facet joints are small joints at the back of the spine. When they are the source of pain, a facet joint (medial branch) block can help confirm it and show whether a longer-lasting treatment is likely to help.

Facet joint pain is typically an ache in the back or neck, often worse when leaning back, twisting or standing for long periods, and stiff in the morning. Scans cannot reliably show whether a facet joint is painful, so a diagnostic block is used.

In line with NICE guidance, medial branch blocks are used mainly as a test before radiofrequency denervation.

Overview

How facet joint injections work

Medial branch block

Local anaesthetic placed next to the small medial branch nerves that supply the facet joints. If your usual pain eases for a few hours, the facet joints are likely to be the source.

Next step: radiofrequency denervation

If a block gives good but short-term relief, denervation may offer longer-lasting benefit by interrupting the same nerves.

Common questions

Frequently asked questions

Why is the relief only short-term?

The block uses local anaesthetic to test the nerves. Short-term relief is expected and shows whether the facet joints are the source of pain.

How many blocks will I need?

Usually one or two diagnostic blocks before denervation is considered.

Do I need a GP referral?

A GP referral is welcomed but not always required. Some insurers need one, so check with your insurer if you are claiming.

Can I use my private health insurance?

Yes. Dr Salama is recognised by most major private medical insurers and is Bupa Fee Assured. Check with your insurer whether you need pre-authorisation. Self-funding patients are also welcome, and consultation fees are published on this site.

How quickly can I be seen?
