Burning, tingling or shooting pain in your feet?

Pain from peripheral and diabetic neuropathy feels different from other pain: burning, shooting, pins and needles, or pain from a light touch such as bedsheets. If it has not settled with standard treatment, or side effects have been a problem, a specialist assessment can help.

Peripheral neuropathy is damage to the nerves outside the brain and spinal cord. It most often affects the feet and lower legs, and diabetes is the most common cause in the UK. Nerve pain can also follow shingles, surgery or injury, or chemotherapy.

For pain in a defined area of skin, such as the feet, one option is a high-strength medicated patch applied in clinic. It is a prescription-only treatment that is not taken by mouth, so it can suit people who have had side effects from nerve pain tablets. Whether it is suitable for you is decided at your assessment.

Getting help

When to see a specialist

Consider a specialist opinion if you have:

  • Burning, tingling or shooting pain in the feet, legs or hands lasting more than three months
  • Neuropathy pain not controlled with standard treatment, or side effects stopping you taking it
  • Pain affecting your sleep, walking or work
  • Pain in a defined patch of skin after shingles, surgery or injury

If you have diabetes and notice a foot wound, a colour change, swelling or a hot foot, contact your GP or diabetes foot team the same day.

Treatment

Treatment options

Treatment starts with confirming the likely cause and making sure the basics are in place, including diabetes control and foot care with your GP or diabetes team. Specialist treatments may be considered when first-line treatment has not helped enough. Not everyone is suitable for every treatment, and results vary.

High-strength medicated patch

A prescription-only patch is applied to the painful skin in clinic: 30 minutes on the feet, or 60 minutes elsewhere, with the skin numbed first if needed. Up to four patches can be used at once. Any benefit usually develops over 1 to 3 weeks and, in people who respond, can last for several months. Treatment can be repeated, usually no sooner than every three months.

Peripheral nerve block

For pain in the area supplied by a specific nerve, an ultrasound-guided injection of local anaesthetic, sometimes with steroid, around that nerve.

Pulsed radiofrequency to a peripheral nerve

In selected people who respond to a nerve block, pulsed radiofrequency may give longer-lasting relief without damaging the nerve.

Common questions

Frequently asked questions

What does the patch treatment involve?

The skin is cleaned and dried, and may be numbed first. The patch is placed on the painful area for 30 minutes on the feet or 60 minutes elsewhere, then removed and the skin cleaned with a special gel. You can go home the same day.

Does it hurt?

Redness and a burning or stinging feeling in the treated skin are very common during and shortly after treatment, and usually settle within a few days. Numbing cream and cooling can help. Blood pressure is checked during treatment because pain can raise it temporarily.

How soon will it work, and how long does it last?

Any improvement usually develops over 1 to 3 weeks. Not everyone responds. In people who do, the benefit can last for several months and treatment can be repeated.

Is it suitable for diabetic neuropathy?

It can be used for painful diabetic neuropathy of the feet, provided the skin is intact. It does not treat the underlying nerve damage, so good diabetes control and regular foot checks remain important.

Who is it not suitable for?

It is not used on the face or head, on broken or irritated skin, in people under 18, or if you are allergic to its ingredients. If your blood pressure is not well controlled, this will be discussed with you first.
